Residential care at Terrance House provides comprehensive, round-the-clock support within a structured care environment. Supported living at House 21 offers greater independence within a more autonomous setting, while still providing tailored daily support based on individual needs.
Individuals who have developed greater independence in residential care and are ready to build further life skills in a supported environment may be suitable. Each placement is assessed individually to ensure the move is appropriate and safe.
Transitions are carefully planned and person-centred. The individual, their family and professionals are involved in the process. Support plans are reviewed, risk assessments completed, and a gradual introduction arranged to ensure continuity and stability.
Support is adjusted, not removed. The level of assistance reflects the individual’s current needs and goals. As independence grows, support can be gradually adapted to encourage continued development.
Support focuses on developing everyday living skills such as managing personal care, preparing meals, budgeting, attending appointments, maintaining a home and engaging with the local community.
Because House 21 and Terrance House work closely together, care pathways can be reviewed if needs change. The priority is always the individual’s well-being and safety.
Progress is monitored through regular support plan reviews, goal setting and outcome tracking. Achievements are discussed with the individual and, where appropriate, with family members and professionals.
